Prescription Survey
- Take a short survey designed for quality improvement efforts by Prescription Turf
- To begin the customer will need the 4-digit Local Prescription Turf Branch Number l
- The Prescription Turf survey number can be found on the customers statement or invoice
- All feedback will be used to improve future customer experiences with Prescription Turf team
- Date in the survey will be collected by the InMoment survey firm
After providing the 4 digit survey branch number the customer will need to locate the Customer ID located on the remittance stub or statement. Customers will be asked to answer a variety of questions rating from 10 which translates to “Completely Satisfied” and 0 which translates to “Completely Dissatisfied,”.
