PetSmart Feedback Survey

• Complete a short survey related to a visit to PetSmart
• In return for their time customers should receive a coupon that can be used on a future visit to PetSmart (to obtain the coupon the survey MUST be completed in full)
• To being the 16 digit PetSmart Feedback pin number will be required
• This number can be foun don the paper receipt received at the time of purchase
• The Service Management Group is responsible for ALL information gathered during the course of the feedback

The PetSmart customer survey will help improve the quality of the customer service and the overall happiness of the pets kept at the store.  Please note the survey is available in English or the Spanish language.  Even customers taking the PetSmart Feedback in the Spanish should be eligible to receive a coupon at the end of the survey.

• Internet connection along with the 16 digit survey number is REQUIRED
• A valid email address will be requested to obtain the PetSmart coupon
• Please answer all the survey questions honestly and candidly
• The PetSmary feedback coupon code will be sent to the customers email address
• Please present the coupon to the PetSmart cashier on a future visit to redeem the promotion
