Meijer Tell Meijer RX

• Take a short survey related to a visit to the pharmacy at Meijer
• The survey code from the pharmacy receipt will be needed
• Tell Meijer RX survey should only take about 5 minutes to complete
• Survey feedback will be collected by the Service Management Group and distributed to the pharmacy managers
• Customers can also take the survey by calling the toll-free number on the Meijer receipt

Customers must take the Tell Meijer RX survey within 72 hours of the purchase or the survey code will become void.  Please note STORE NUMBER 28 in Flint Michigan will not offer the survey to RX customers.  Customers who do complete the survey in full will become eligible to participate in the Meijer Feedback survey sweepstakes.

• Customers must be 18 years of age or older
• No purchase is required to enter
• Residents in the states of Rhode Island, Florida and New York are not allowed to participate in the Meijer Tell Meijer RX SWEEPSTAKES due to deranged state laws
• 12 $1000 GIFT CARDS will be given away in total
• The total value of the Meijer RX survey sweepstakes is $12000
• Odds of winning will be based on how many people visit the pharmacy and end up taking the survey
• The $1000 Meijer gift card draw will take place around the 1st of each month for the previous month
• The gift card CANNOT be used when making a purchase at the Meijer pharmacy for some strange reason

To enter the promotional sweepstakes without purchase please write to: “Meijer Attn: Customer Contact Center, 2727 Walker Ave, NW, Grand Rapids MI 49544”.  The contestants should include a nifty 3×5 index card with their name and contact information clearly printed in ENGLISH.  This method of entry will provide the contestants with ONE (1) entry into the Meijer Feedback Survey sweepstakes.  Kent County Courts in the state of Michigan will have jurisdiction over any legal battles that arise due to this promotion.
